Come Join the Seasons Team:Seasons Hospice & Palliative Care is seeking a Patient Family Liaison to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.Overview:The Patient Family Liaison is responsible to sign-on hospice appropriate patients onto the Seasons Programs. This discipline is also responsible for the facilitation of referrals, pending admissions, informational and election of benefits visits to increase access to care.Qualifications:Meets hospice licensure requirements in the state of California for any of the following disciplines:
MSW - Social Worker

  • Master's degree required
  • Qualifications for state-specific requirements above Seasons minimum education standards are as follows:
  • 1+ year experience required; 2+ years preferred
    a. Palliative only does not require experience
  • Medicare/Medicaid/Insurance specialty preferred
  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance
  • Additional Qualifications for Pre-Hospice Social Worker: Experience with coordination of insurance benefits (such as a Medicaid Specialist) preferredRegistered Nurse
    • Associates degree, preferred
    • Registered Nurse license required in state(s) practicing
    • Current and valid CPR certification required in California
    • Minimum one year of experience required in California
    • Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.
    • Willingness to serve on-call
    • Must have reliable automobile, valid driver's license, and the minimum state required liability auto insurance
    • Meet the regulations and requirements of the State in which the program provides servicesLVN
      • Associates degree, preferred
      • Current and valid Licensed Vocational Nurse license in state(s) license(s) where practicing required
      • Current and valid CPR certification required in California
      • Meet the regulations and requirements of the state(s) in which program provides services
      • Minimum one year of experience working in a licensed field required in California
      • Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.
      • Must have reliable automobile, valid driver's license, and the minimum state required liability auto insuranceChaplain
        • Bachelor's Degree from a Council for Higher Education accredited college, university or divinity school is required; a graduate-level master's degree from a similarly accredited college, university or theological school preferred.
        • Ordained, credentialed, or commissioned according to the practices of a legitimate and recognized religious organization/faith tradition.
        • A minimum of one Clinical Pastoral Education (CPE) unit, two or more, preferred. Equivalencies for CPE will be granted per established guidelines of individual professional organizations. Prior experience in hospice also preferred.
        • Minimum of one year supervised experience in a medical, educational or religious agency/institution.
        • Ability to provide an inter-faith approach to spiritual care demonstrating a high level of cultural and theological competence in working with diverse faith and belief systems.
        • Demonstrate excellent oral and written communication skills, presentation skills, and teaching skills.
        • Computer skills: Word, email, patient database charting, internet, voicemail.
        • Must have a reliable automobile, valid driver's license, and the minimum state required liability auto insurance.
          Preferred three years' experience in healthcare agency or population.Meet the regulations and requirements of California state.Demonstrates strong clinical skills.Demonstrates excellent verbal and written communication skills.Demonstrates commitment to hospice philosophy of care.Demonstrates the ability to work closely with a variety of people effectively using problem solving skills.Demonstrates a willingness to learn regulations and related reimbursement policies for hospice and long-term care.Demonstrated commitment to AccentCare Hospice's philosophy of care, values, mission statement and organizational culture.
