Location: Ontario, CA Description: 1. Look at existing tasks for
f/u and case work needed.--2. Review chart files to obtain talking
points based on what is found in the HRA, Annual Health Assessment,
authorizations in UM system for new patients and those due for
annual reassessment.--3. Outreach the patient and discuss the
alerts/triggers from the clinical reviewed to identify needs for
Coordination of care, and goals the patient would like to work
on.--4. Create a care plan, or update existing care plan if one has
been created w/in the year.--5. Present patient in weekly rounds
(average of two patients presented weekly) and document in
appropriate assessment. What software skills are needed? Outook,
Nammnet Express, Care Director (Care in Motion) are the primary
apps used regularly.--Training will be given for all but outlook.
What are the hours? ( Training hours?) 08:30-17:30 are work hours.
